What is the recommended preparation method for a phenytoin cream intended for Sudeck's syndrome, given that the basis was not specified, it is the first time the physician is prescribing it following a specialist's recommendation, and while the APB suggested using a PEG or Natrosol gel basis, the latter is unavailable, there is no existing protocol for this preparation in Belgium, gabapentin cream or ointment was suggested as an alternative, and a formula developed by a Dutch pharmacy in collaboration with a neuropathic pain center is patented?

Proposal for a Cream with Phenytoin

Proposing a cream with scientific evidence for efficacy in literature is complex. Although two creams with 5% phenytoin are described, they often utilize cream bases for which only the brand name is known and the exact composition is missing.

A more accessible alternative is a preparation with 5% phenytoin in hydrophilic ointment USP. The composition of this base is as follows:

  • Methylparaben: 25 mg
  • Propylparaben: 15 mg
  • Sodium lauryl sulfate: 1 g
  • Propylene glycol (or glycerin): 12 g
  • Cetyl alcohol: 25 g
  • Petrolatum: 25 g
  • Water: up to 100 g
